Day- 1 Report

Department of Shalyatantra, D. Y. Patil deemed to be University, School of Ayurveda,Nerul, Navi Mumbai, had organized “National Symposium on Integrative Proctology: A 6 Day CME Programme” from 17^th October 2023- 22^nd October 2023.

DAY 1 –17th October 2023, Tuesday.

SessionCoordinator: Dr Nisha Thadani (Assistant Professor Shalyatantra Department).Inaugural ceremony started with the welcome of our dignitaries and all delegates in the virtual classroom by Dr Nisha Thadani madam AssistantProfessor of Shalyatantra department, followed by Dhanvantripoojanand deep prajawalan by our honourable Registrar Alka Kulkarni madam, D.Y. Patil Deemed to be University, Navi- Mumbai, Guest of Honor Dr V. P UkhalkarSir Professor and Head of Shalyatantra Department at R. A. Podar Government Medical College, Worli, Mumbai, Dean Dr Maheshkumar Harit Sir School of Ayurveda,D.Y. Patil Deemed to be University, Navi- Mumbai, Dr Amarprakash Dwivedi Organising Chairman, Professor and Head of Shalyatantra Department, Dr Mayur Pawaskar Organising Secretory and Associate Professor atShalyatantra Department, Dr Vedika Bhoir Co-Organising secretory andAssociate Professor atShalyatantra Department,Guest speakers DrManisha Nakhate MadamProfessor and Head of Anatomy Department, School of Medicine, D Y Patil deemed to be University, Navi- Mumbai.

After that felicitation programme was held, our Dean Sir felicitated our chief guest Honourable Alka Kulkarni Madam. Our Organising Chairman Dr Amarprakash Dwivedi sir felicitated our guest of honour Dr V. P Ukhalkar Sir, then our Organising secretary Dr Mayur Pawaskar sir felicitated our Dean Dr Maheshkumar Harit Sir, Dr Mayur Pawaskar sir felicitated our Organising Chairman Dr Amarprakash Dwivedi sir. Our Registrar Alka Kulkarni madam shared her insights, after that Our guest of honour Dr V. P Ukhalkar sir shared his insights, our dean sir shared few words about six-day CME Programme. After that our Organising Secretory Dr Mayur Pawaskar Sir delivered his welcome address, and lastly our Organising Chairman Dr Amarprakash Dwivedi sir declare the opening of scientific session.

Session :1

Lecture 1: Anatomical consideration in Proctology

Time: 9:30am to 10:30am.

The first guest lecture was conducted bySpeaker Dr Manisha Nakhate madam, Professor and head of Anatomy Department, School of Medicine, D Y Patil deemed to be University, Navi- Mumbai. Madam explainedabout anatomy of the rectum and anal canal in details. Post lecture madam was felicitated by Dr Mayur Pawaskar sir.

Lecture 2: Management of constipation in Ayurveda.

Time: 10:30am to 11:30 am

The second lecture was conducted by our guest speaker Dr Pradnya Kapse madamAssistant Professor at R. A. Poddar Government Medical College, Worli,Mumbai. Madam has given brief introduction in Management of constipation in Ayurveda, she also explained about obstructive defecation syndrome along with STARR procedure. It was very informative lecture. Post lecture Dr Pradnya Kapse Madam was felicitated by Dr Vedika Bhoir madam.

Lecture 3: Clinical Examination in proctology and constipation management

Time: 12.00 pm to 01.00 pm

The third lecture was conducted by Dr V. P UkhalkarProfessor and HOD of Shalyatantra Department at R. A. Podar Govt. Medical College, Mumbai. In the lecture, Sir had explained Clinical Examination in proctology including MRI defecogram, Anorectal Manometry etc. Sir also explain constipation, various constipation scoring and its managementaccordingly. The whole lecture was very informative. Post lecture sir was felicitated by Dr Amarprakash Dwivedi sir. After that panel discission was done with question answer session was conducted and moderated by Dr Mayur Pawaskar Sir.

Post Day 1 session Pre and Post Evaluation forms and feedback form were collectedfrom all the delegates. Session ended with vote of thanks to the founder of D.Y. Patil University Honourable President Dr D. Y. Patil Saheb, Honourable Chancellor and President Dr Vijay D. Patil Sir, Honourable Pro Chancellor and Vice President Mrs. Shivani Patil Madam, Honourable Vice Chancellor Dr Vandana Mishra Madam, all dignitaries, all delegates and whole organizing committee of the CME Programme by our co-ordinator Dr Nisha Thadani Madam followed by National Anthem. The Day1 of CME Programme was attended by all registered  delegates including general practitioners from all over Maharashtra and many other states faculty members, undergraduates’ students, postgraduate students, Interns of all departments. The first day session was primarily aimed to have brainstorming session on the basic fundaments of “Integrative Proctology”.

Day- 2 Report

Session Coordinator: Dr Abhinav Dubey (1^st Year PG Scholar Dept of Shalya tantra). Dr Abhinav Dubey had Welcomed all the respected dignitaries and delegates. Session started with the blessings of lord Dhanvantri and deep prajawalan by our Organizing Secretory and Associate Professor in Shalyatantra Department D.Y. Patil Deemed to be University, School of Ayurveda Dr Mayur Pawaskar Sir, Dr Santosh Chavan sir, Professor & Head of Department Rog Nidan, Dr Amol Sir, Deputy Additional Medical Superintendent D Y Patil Ayurvedic Hospital, Dr Nisha Thadani our CME Co-ordinator and Assistant Professor Shalyatantra Department.

Session 1: Live Surgical Demonstration     

Case 1: Chronic Fissure in Ano with Sentinel Tag

Time: 09:00 am to 10:30 am

Live surgical demonstration was performed by Dr Vedika Bhoir Madam Associate Professor D Y Patil School of Ayurveda, Navi Mumbai. Dr Mayur Pawaskar sir was the moderator for the session. Madam performed justify lateral sphincterotomy with excision of sentinel tag under spinal anaesthesia and explained all important steps of the surgery. After completion of surgery case discussion and Question answer session was done by Dr Vedika Bhoir and Dr Mayur Pawaskar sir. Various questions regarding post operative wound care and LIS were discussed.

 

Session :2

Lecture 1: Recent Advancement in management of Fissure in Ano

Time: 10:30 am to 11:30 am

The first guest lecture was conducted byguest Speaker: Dr Anuradha Panchal MadamProfessor & HOD, Department of General Surgery, School of Medicine, D Y Patil Deemed to be University, Navi- Mumbai. Madam explained in details about Fissure in Ano and various Conservative & Surgical management for the same such as Fissurectomy, Sphincterotomy, Lords Anal Dilatation, LASER Sphincterotomy etc. After lecture question answer session was conducted. Dr Anuradha Panchal madam was felicitated by Dr Mayur Pawaskar sir.

Lecture 2: Ayurvedic prospective of Fissure in Ano management.

Time: 11:30am to 12:30 pm

The second lecture was conducted bySpeaker Dr Madhuri Bhandare Madam, Associate Professor, R A Poddar Ayurvedic medical college and Hospital, Worli, Mumbai. Madam explained relation of lifestyle with Fissure in Ano, pathya and apathya in Fissure in Ano, treatment for malabadhata, upadravachikitsa, post operative management in ayurveda, research study of various types of tail poorana, tail pichu dharana. It was very informative lecture. Post lecture question and answer session was conducted, delegates asked various questions regarding conservative ayurvedic management of Fissure in Ano. After that our guest speakerDr Madhuri Bhandare Madam was felicitated by Dr Vedika Bhoir madam.

Session 3: Panel Discussion and Question Answer session

Time: 12:30 pm to 1:30 pm-

Dr. Mayur Pawaskar sir moderated the panel discussion and question answer session. In this session all delegates ask questions related to the lectures and various complications of the diseases and office procedures were discussed. The panelists were Dr Madhuri Bhandare madam,Dr Mayur Pawaskar Sir and Dr Vedika Bhoir madam.It was an interactive session with delegates.

Session 3: Scientific Paper Presentation

Time: 1:30pm to 2:30 pm

Scientific paper presentation session was done by the delegates. This session was moderated by Dr Mayur Pawaskar. After the presentation certificates were distributed to all thepresenters.

Post day 2 session Pre and Post Evaluation forms and feedback form were collected from all the delegates. Session ended with the vote of thanks by Dr Abhinav Dubey.

Sir performed sclerotherapy with justify lateral sphincterotomy with excision of sentinel tag under spinal anaesthesia and explained all important steps of the surgery. After completion of surgery case discussion and Question answer session was done by Dr Dynaneshwar Chavan sir and Dr Mayur Pawaskar sir. Various questions regarding post operative wound care and complications were discussed. Later on, Dr Dyaneshwar Chavan sir was felicitated by Dr Mayur Pawaskar sir.

Lecture 1: Recent advancement in Haemorrhoid management

Time: 10:30 am to 11:30 am

The first guest lecture was conducted byguest Speaker: Dr Kailash JawadeProfessor, Department of Surgery, School of Medicine, D Y Patil Deemed to be University, Navi- Mumbai. Sir has explained brief about haemorrhoids and various treatment available as rubber band ligation, Foam sclerotherapy, cryotherapy, phototherapy for haemorrhoid, laser hemorrhoidopexy, MIPH stapler haemorrhoidectomy etc. After lecture question answer session was conducted. Later the session Dr Kailash Jawade sir was felicitated by Dr Vedika Bhoir madam.

Lecture 2: Ayurvedic prospective of Haemorrhoid management.

Time: 11:30am to 12:30 pm

The second lecture was conducted bySpeaker Dr.Amit PaliwalProfessor, D.Y. Patil Ayurveda college, Pune. Sir has explained relation of lifestyle with haemorrhoids, pathya and apathya in Arshas, treatment for malabadhata, upadravachikitsa, post operative management, research study of various types of kshara application, complications of kshara application, management of arshas by agnikarma etc. It was very informative lecture. Post lecture question and answer session was conducted, delegates asked various questions regarding conservative ayurvedic management of haemorrhoids. After that our guest speaker Dr Amit Paliwal sir was felicitated by Dr Mayur Pawaskar sir.

Session 3: Panel Discussion and Question Answer session

Day 4 – Report

DAY 4- 20^th October 2023, Friday.

Session Coordinator: Dr Damini Raut (1^st Year PG Scholar Shalyatantra Department). Dr Damini Raut welcomed all the respected dignitaries and delegates. Session started with the blessing of lord Dhanvantari and deep prajawalan by our Organizing Secretory and Associate Professor in Shalyatantra Department D.Y. Patil Deemed to be University, School of Ayurveda Dr Mayur Pawaskar Sir, Dr Vedika Bhoir our Co-organizing Secretory and Associate Professor in Shalyatantra Department D.Y. Patil Deemed to be University, School of Ayurveda.

Session 1: Live Surgical Demonstration

Case 1: Fistula-in-Ano with Grade 2 internal Haemorrhoids.

Time: 09:00 am to 10:30 am

The Resource Person for case 1 live surgical demonstration was Dr Sayaji Desai sir Professor at YaswantAyurved College, Kodoli.Case was done in spinal Anesthesia. PPD done under all aseptic precautions. Sir started the case with P/R examination and 2 fingers lord’s anal dilatations. For fistula in Ano sir did complete fistulectomy and in proctoscopy examination patient was having grade 2 internal hemorrhoids so Sclerotherapy done at 3 and 7 ‘o’ clock position. Procedure went uneventful. Sir explained all the steps of surgery in details.

Case 2: Fistula-in-Ano.

Live surgical demonstration of 2^nd case was done by Dr Amarprakash Dwivedi sir, case was of A male patient aged 34 years having small fistula in ano with external opening seen at 6 ‘o’ Clock position 2 cm away from anal verge, internal opening was traced at 6 ‘o’ Clock position. Under spinal anesthesia PPD done under AAP. Sir did ksharsutraligataion for the fistula in ano. In this live surgical demonstration sir explained complete information about the surgical procedure.

Dr. Mayur Pawaskar sir has moderated the whole live surgical demonstration session. After completion of surgery Dr. Sayaji Desai sir, Dr. Amarprakash Dwivedi sir, Dr. Nandkishor Borse sir and Dr Mayur Pawaskar sir had discussed the case followed by question answer session regarding the surgery. Later on, Dr. Sayaji Desai sir was falicitated by Dr. Amarprakash Dwivedi sir and Dr. Nandkishor Borse sir was falicitaed by Dr. Mayur Pawaskar sir.

The first guest lecture was conducted by guest Speaker: DrBibekananda MahapatraAssociate Professor at Department of surgery at School of Medicine, D Y Patil University Deemed to be university Navi- Mumbai. In the lecture sir had explained brief about Recent advancement in fistula management like laser therapy, Fibrin glue, Fibrin plug, LIFT procedure, VAAFT surgery etc. All complications were discussed regarding few surgical procedures. After lecture question answer session was conducted. Later the session Dr Bibekananda sir was felicitated by Dr Amarprakash Dwivedi sir.

Lecture 2:Ayurvedic prospective of fistula in Ano management

Time: 11:30am to 12:30 pm

The second lecture was conducted by guest Speaker: Dr Mahesh Sanghavi (Renowned Ayurveda Proctologist). Sirhas given detailsabout of fistula in Ano management in Ayurveda it was very informative lecture. Sanghavi Sir has explained about the IFTAK (interception of fistulous tract and application of Ksharsutra) procedure for fistula in Ano management, Effects of ksharsutra etc. Sir also shared many complicated high anal fistula case reports, complex and recurrent fistula in ano management. Sir also discussed various complications regarding many anal anal fistula surgeries. Post lecture question and answer session was conducted, delegates asked various questions regarding the lecture. After that our guest speaker Dr Mahesh Sanghavi sir was felicitated by Dr Amarprakash Dwivedi sir and Dr Vedika Bhoir madam.

This case was operated along with live demonstration done by Dr Amarprakash Dwivedi Sir. Dr Mayur Pawaskar Sir was the moderator for the session. Sir performed excision of Pilonidal sinus under local anaesthesia and explained all important steps of the surgery. After completion of surgery case discussion and Question answer session was done by Dr Amarprakash Dwivedi sir, Dr Nilesh Doshi sir and Dr Mayur Pawaskar sir. Various questions regarding case1 and case 2 including post operative wound care and complications were discussed.

 

Case 2: Pilonidal sinus

Time: 10:00 am to 11:00 am

The Resource Person for live surgical demonstration of case 2 was Dr Nilesh Doshi Sir. He is a renowned proctologist at Devangi Hospital, Malad, Mumbai. Dr Mayur Pawaskar Sir was the moderator for the session. Sir performed Partial excision of Pilonidal sinus with Ksharsutra ligation under spinal anaesthesia and explained all important steps of the surgery. After completion of surgery case discussion and question answer session was done by Dr Nilesh Doshi, Dr Amarprakash Dwivedi and Dr Mayur Pawaskar sir. Various questions regarding post operative wound care and complications including recurrence were discussed in details.

Lecture 1: Recent advancement in Pilonidal sinus management

The first guest lecture was conducted by guest Speaker: Dr Anuradha Panchal madam Professor and HOD, Department of General Surgery, School of Medicine, D Y Patil Deemed to be University, Navi- Mumbai. Madam has explained details about pilonidal sinus and various treatment modalities available such as Z Plasty, Karydakis excision, Limberg flap excision, Bascom technique, Lahey and Cattells method, Laser Pilonidoplasty,VAAPS, EPiST.After lecture question answer session was conducted. Dr Anuradha Panchal Madam was felicitated by Dr Amarprakash Dwivedi Sir and Dr Mayur Pawaskar Sir.

Lecture 2: Ayurvedic prospective of Pilonidal sinus management.

The second lecture was conducted by Speaker Dr Nilesh Doshi Sir renowned proctologist Devangi hospital, Malad Mumbai. He had explained ayurvedic prospective of relation of pilonidal sinus that is Agantujnadivrana, hetu, purvaroop, roop, samprapti, upashyanupashya post operative wound management, various research study. It was very informative lecture. Post lecture question and answer session was conducted, delegates asked various questions regarding ayurvedic management of pilonidal sinus with ksharasutra ligation. After that our guest speaker Dr Nilesh Doshi sir was felicitated by Dr Amarprakash Dwivedi and Dr Mayur Pawaskar sir.

Lecture 3: Integrative proctology

Time: 11:30am to 12:30 pm

The third lecture was given by Dr Amarprakash Dwivedi Sir Professor and HOD, Department of Shalyatantra, School of Ayurveda, D Y Patil Deemed to be University, Navi- Mumbai. Sir had elaborated ayurvedic and modern perspective of various diseases of proctology like fissure in ano, fistula in ano, haemorrhoids, anorectal abscesses, anorectal prolapse and its management, research study and recent advancement in proctology. It was very informative lecture. Post lecture question and answer session was conducted, delegates asked various questions regarding integrative proctology. After that Dr Amarprakash Dwivedi Sir was felicitated by Dr Mayur Pawaskar Sir, Dr Vedika Bhoir Mam and Dr Nisha Thadani Madam.

For this session Resource person was Dr Sheetal Asutkar Madam Professor and Head of Shalyatantra Department at Mahatma Gandhi Ayurveda College Hospital and research Centre, DMIHER (Datta Meghe Institute of Higher Education and Research), Sawangi, Wardhaand the session was moderated by final year PG Scholar Dr Zeeshan Ansari.

Post day 5 sessions, Pre and Post Evaluation forms and feedback form were collected from all the delegates. Session ended with the vote of thanks by Dr Kalyani Bhiwgade followed by National Anthem.

Guest Lecture 1: Management of Anorectal Diseases in Ayurveda

Time: 9:30 pm to 11 a.m.

The first session was conducted by the welcome of our Guest Speaker Dr Sheela Kohad madam, Head of Shalyatantra DepartmentA.V.P.M Ayurved College, Sion, Mumbai.Madam enlightened the students with her lecture on the Basic fundamentals of “Management of anorectal diseases in Ayurveda”. Madam also shared few of her clinical cases managed by various ayurvedic procedures like matrabasti, kshar applications, ksharsutra ligation etc. she also showed some video clips of thematrabasti procedure, and explained each steps thoroughly. Then the question answer session was conducted for the lecture. After completing that, the felicitation of our guest speaker was done by our Guest of Honor for the day Dr Yonus Solanki sir Professor at R. A Podar Government Ayurvedic College from Worli, Mumbai, our Organizing Chairman and HOD Shalyatantra Department Dr Amarprakash Dwivedi sir and our Organizing Secretory Dr Mayur Pawaskar sir.

Lecture 2: My Clinical Experiences in Proctology.

Time: 11:00 am to 12:30 pm.

The Guest Speaker for this session was respected Dr Yunus Solanki Sir, Professor, Department of Shalyatantra, R.A. Podar Government Medical College, Worli,Mumbai. Sir started his guest lecture on Clinical experiences in the field of Proctology. He shared various difficult cases as complex and recurrent fistula in ano, high anal fistula cases and rare cases of anorectal malformation as imperforate anus of a newborn. He also shared his various National and International conferences experiences with many eminent personalities in the field of Proctology. It was indeed a knowledgeable session for all the delegates, Faculty, PG and UG students. After his lecture question answer session and panel discussion was done, it was an interactive session with the delegates.

Lecture 3: Herbs in the management of haemorrhoids.

The Guest Speaker for this session was Dr Manisha Mishra Madam. Madam is the Senior Medical advisor in the Charak Pharma Pvt Ltd. Madam Started her Lecture with the details of various Herbs and their formulation usually used in the conservative management of various anorectal diseases including per rectal bleeding, haemorrhoids and constipation etc.

Session 2: Valedictory Function

The session started with the welcome of our chief guest Honourable Dr Vandana Mishra Madam Vice Chancellor D. Y. Patil Deemed to University, Nerul, Navi Mumbai, Dr Mahesh Kumar Harit Sir Dean D. Y Patil Deemed to be University, School of Ayurveda, Nerul, Navi Mumbai. All the dignitaries started the last session of our whole CME event with lord Dhanvantari blessings and prayer. Dr Mayur Pawaskar Sir Organizing secretory summarized the whole event. After that felicitation programme was held, our Dean Sir felicitated our chief guest Honourable Dr Vandana Mishra madam and Guest of Honor Dr Yonus Solanki Sir. Dr Mahesh Kumar Harit sir was felicitated by our Organizing Chairman Dr Amarprakash Dwivedi sir. Dr Mayur Pawaskar Sir felicitated Dr Amarprakash Dwivedi sir. Dr Manisha Mishra mam Senior Medical advisor in the Charak Pharma Pvt Ltd and DrAnand Jalukar Managing Director, Vidyanand Lab Pvt Ltd. were felicitated by Dr Amarprakash Dwivedi Sir. Dr Vandana Mishra madam, Dr Yonus Solanki and Dr Mahesh Kumar Harit sir shared their insights on the CME. Lastly Dr Amarprakash Dwivedi gave the valedictory speech.

Post Day 6 session Pre and Post Evaluation forms and feedback form were collectedfrom all the delegates. Session ended with vote of thanks to the founder of D.Y. Patil University Honourable President Dr D. Y. Patil Saheb, Honourable Chancellor and President Dr Vijay D. Patil Sir, Honourable Pro Chancellor and Vice PresidentMrs. Shivani Patil Madam,Honourable Vice Chancellor Dr Vandana Mishra Madam, all dignitaries, all delegates and whole organizing committee of the CME Programme by our co-organizing secretory Dr Vedika Bhoir Madam followed by National Anthem.

The whole CME Programme was registered by 100 delegates and attended by around 200 delegates including general practitioners from all over Maharashtra and many other states faculty members, undergraduates’ students, postgraduate students, Interns of all departments. The event was primarily aimed to have brainstorming session on “Integrative Proctology” used in the management of various anorectal diseases.
